This agreement is between Trident Healing and the named participant. To lay the foundation of our business relationship, I want to provide you with the following information for our work together.
Fees
(total cost will be prorated based upon time spent per session):
• Individual work - $150/hour (one-hour session
is 55 minutes)
• Couples work - $200/hour (one-hour session is 55 minutes)
• Group work - $300/month
Procedure for sessions:
Client and coach will connect by phone, Zoom, or “other” form at a pre-arranged time.
Arrangements for payment must be made prior to session.
Changes:
If you need to reschedule an appointment, please allow twenty-four hours (24) hours notice. If 24 hours notice is not received, the session will be counted and payment for the session shall be expected. Unforeseen situations will be addressed on a case by case basis.
Responsibilities of Trident Healing:
Trident Healing will honor our relationship with the highest degree of professionalism, integrity, and give you the honesty you deserve. I will fully respect you confidentiality and will be committed to your success. I will reflect and model the joy of life in our relationship and create the necessary space for healing. Trident Healing abides by the ICF (International Coaching Federation) Code of Ethics.
Responsibilities of Client:
Client(s) understand that coaching is an ongoing relationship that is meant to produce fulfilling results in their personal and professional lives. Client understands that while coaching can be incredibly satisfying, it does take effort on my(our) behalf during and between sessions. Growth will be directly connected to the level of commitment throughout this process. Within this relationship, the client expects to deepen learning, improve performance, and enhance quality of life. I(we) take full responsibility for all our actions and results. I(we) will not hold Trident Healing liable for any consequences, as all decisions will be made by the client. Should client feel value is not being received, feel at ease to discuss with coach. I(we) will be punctual, accountable, and committed to results.
Understanding of the program:
Client’s confidentiality is ensured and information will not be released without informed consent, except in cases where a child’s safety, well being, or the safety of others is at risk. It is understood that a coach does not act as a counselor, therapist, minister, or other expert in our life. Client is aware that the coaching relationship is in no way to be construed as psychological counseling or any type of psychotherapy. Coaching sessions are not a replacement for existing supervisory or management processes.
Client enters into a coaching relationship with Trident Healing with the understanding that he or she is responsible for creating his or her own decisions and results. Client also agrees not to hold Trident Healing liable for any actions or results for adverse situations created as a direct result of a specific referral given.
Termination:
Client may end this agreement at any time this relationship no longer serves their needs. Contract fees which have been paid for future services shall be addressed on a case by case basis.
By signing here, I(we) acknowledge that we have read and understood this contract agreement. We declare that we will honor our commitment as clients and understand our coaching arrangement. We hereby agree to all mentioned terms of the agreement.
